Change of Beneficiary Form
Changing a beneficiary(ies) is an important decision that should be made with care. Click here to read helpful hints about making a beneficiary change.
Service Request Form
This form is required for all name change requests, address changes, cash surrenders, duplicate certificate requests, certificate loans and more.
Absolute Assignment Form
The Absolute Assignment of Certificate Form is used to transfer ownership on life insurance certificates. The form must be completed, signed by the policy owner and witnessed by someone other than the beneficiary and/or new owner(s). Please note: Ownership transfers dramatically change the dynamics of a certificate. No activity can occur, even simple questions answered, without the written approval of the assignee.
Collateral Assignment Form
This form is often used when assigning a non-qualified annuity or life insurance policy to a funeral home. Contact your local Sales Representative for more information on the correct use of this form.

Member Assistance Programs
The storms of life are unpredictable in all ways but one...they will happen.
When a member is in financial hardship due to traumatic illness, natural disaster or military service, Catholic United has a Member Assistance matching grant program that will match dollars raised, up to $1000, at a fundraiser in the name of the member. Contact a Member Engagement staff member to access this benefit.
The Precious Life Insurance Benefit plan offers members a no-cost benefit for newborn children and a protection option for infants that are not insurable. Conditions apply. Contact your local Sales Representative for more details about this benefit.
